Increased Risk of Heterotopic Ossification Following Revision Hip Arthroplasty for Periprosthetic Joint Infection

چکیده مقاله:
Purpose: To investigate whether surgery for PJI of the hip, the number of procedures and their duration contribute to risk of HO formation. Methods: 56 patients with hip PJI undergoing one-stage (10) or two-stage (46) exchange arthroplasty were matched to 112 patients undergoing revision arthroplasty for aseptic failure based on age, gender, body mass index (BMI), surgical approach (all direct lateral) and date of surgery (2006-2013). Patients with Paget’s disease and ankylosing spondylitis, or preoperative HO were excluded. Perioperative pain management included use of the anti-inflammatory medications in all patients without prophylactic radiotherapy. Six-month postoperative radiographs were reviewed based on Brooker classification. Results: The incidence of overall HO in PJI and aseptic groups was 84% (47/56) and 11% (12/112), respectively. High grade HO (grades 3 and 4) in PJI and aseptic groups were 25% (24/56) and 4% (4/112), respectively. PJI was an independent risk factor for HO in the multivariate analysis (odds ratio of 9.3, 95% CI: 2.9-29.9, p

Revision for prosthetic joint infection following hip arthroplasty
OBJECTIVES We used the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man (NJR) to investigate the risk of revision due to prosthetic joint infection (PJI) for patients undergoing primary and revision hip arthroplasty, the changes in risk over time, and the overall burden created by PJI. METHODS We analysed revision total hip arthroplasties (THAs) performed due t...
متن کاملOutcomes of heterotopic ossification excision following revision hip arthroscopy
This study reviewed pain and outcome scores of patients undergoing revision surgery with heterotopic ossification (HO) excision following previous hip arthroscopy. The aim was to determine if performing the excision arthroscopically improved clinical outcomes. Data were prospectively collected and retrospectively reviewed in patients who had HO removed arthroscopically between February 2008 and...
متن کاملPatient satisfaction after revision hip arthroplasty or resection hip arthroplasty due to periprosthetic infection.
In this retrospective study we have analysed 10-year period results of all type periprosthetic hip joint infection treatments at our Department. Data for 73 patients were analysed and functional status for 41 patients evaluated. A smaller proportion of patients (45%) with resection arthroplasty as the definitive solution were satisfied. These were mostly females with numerous comorbidities and ...
متن کاملRisk factors for periprosthetic joint infection in total knee arthroplasty.
PURPOSE To evaluate the incidence and risk factors of periprosthetic joint infection (PJI) in patients undergoing total knee arthroplasty (TKA) in a high-volume hospital. METHODS Records of 1133 primary TKAs were reviewed. Correlation between surgery volume and infection rate was determined. Risk factors for PJI were identified using case-control analysis of variables. TKAs performed between ...
متن کاملBMI is a key risk factor for early periprosthetic joint infection following total hip and knee arthroplasty.
AIM To identify patient and surgical risk factors that are associated with periprosthetic joint infection (PJI), especially whether obesity is a risk factor following total hip arthroplasty (THA) and total knee arthroplasty (TKA). METHODS New Zealand Surgical Site Infection Improvement Programme data was analysed using deep infection within 90 days of the index procedure as the outcome. This ...
متن کاملSystematic review of risk prediction scores for surgical site infection or periprosthetic joint infection following joint arthroplasty.
Accurate identification of individuals at high risk of surgical site infections (SSIs) or periprosthetic joint infections (PJIs) influences clinical decisions and development of preventive strategies. We aimed to determine progress in the development and validation of risk prediction models for SSI or PJI using a systematic review. We searched for studies that have developed or validated a risk...
